At Okanagan Laser Solutions, we take concussions very seriously as they have the potential to alter physical and cognitive function on a day-to-day level if left unresolved. Below is our clinic’s questionnaire to allow you to assess the severity of your concussion. Mild to moderate concussions score less than 25, and moderate to severe concussions score greater than 25.

Leaving details about the concussion incident and symptoms experienced are not required but would help us know more about your circumstances before coming into the clinic.
